Does anyone know how to go about this if there is a form or something he needs to send to the state. Please help the old man out! I didn't go to college,... shawncat Posted September 13, 2008 Posted September 13, 2008 Call Mo.department of conservation 314-301-1500[i think thats it] and ask for form, then I think your dad has to get his doctor to sign it.If that does not work, post again and I'll call one of my handicap hunter friends.
